The physical, psycho-social, spiritual and cultural needs of the dying and bereaved are met from the time of the onset of life changing illness, through anticipatory grief, at the time of death and continue through to the first anniversary of the death. Support is offered one to one on personal issues of loss, changes in family dynamics and personal contribution, unresolved grief, problem solving, preparation for separation and death. Past death, one to one and introduction to bereavement support groups help the bereaved to find value and purpose in the life and death of the deceased, and their ongoing journey. Peer support, mail contact by newsletters, services of remembrance compliment one to one support. A card mail out system acknowledges anniversaries and sends a strong message of connection and availability. Community education, allied health carer education and debriefing support and networking with other professionals to achieve better bereavement outcomes ensures a continuing sustainable bereavement service to our community. Our services are provided by a co-ordinated effort of Palliative Care Service Co-ordinators, Bereavement Support Workers and Palliative Care Volunteer Support Workers.
